Odyssey Charter School Volunteer Guidelines

Thank you for your willingness to share your time and talents as a volunteer at Odyssey Charter School! Thanks to the contributions from people like yourself, the school can continue to be a success. Your efforts are very much appreciated.

To assure safety of all students and community members, volunteers are required to register with the School as follows:
Print and submit all Volunteer forms and required documents located below.

Required forms:

  1. Volunteer Registration & Disclosure Form View/Print Here
  2. Volunteer Agreement View/Print Here
  3. Volunteer Code of Conduct View/Print Here
  4. Confidentiality Agreement View/Print Here
  5. Background Check; please submit a copy of the receipt to HR, so they know when to expect receipt of your report View/Print Here
  6. Child Registry Form; return completed form to HR. HR will submit your form to the State on your behalf and keep the results on file View/Print Here
  7. TB test results; Volunteers are required to have a TB skin test (Mantoux) or other approved test, such as blood test, administered prior to their assignment and every 5 years thereafter. A test can be administered and read from your primary physician or any freestanding medical aid unit.
  8. TB Questionnaire Form; return completed form to HR View/Print Here

All paperwork is confidential and handled as such under the State of Delaware Regulations and only accessible by official OCS HR representatives.
