Complete Record of Immunizations
Obtained from your child's physician. Required for all students.
Emergency Treatment Card 2025-26
This form authorizes emergency medical treatment if you cannot be reached.

If your child has a specific health condition or needs medication at school, additional forms may be required. Work with your campus nurse to determine which forms apply.
Allergy and Food Allergy
These forms document your child's allergies and the emergency action plan if an allergic reaction occurs.
Asthma and Respiratory
Required if your child uses an inhaler or has asthma. The self-administration form allows older students to carry and use their own inhaler at school.
Medication Administration
Required if your child needs medication during the school day. Complete the appropriate form based on whether the medication is prescription or non-prescription.
Severe Conditions & Action Plans
Emergency protocols for specific severe conditions. The epinephrine form allows the school to support your child in using life-saving medication.
Other / Opt-Out forms
Forms for athletic participation and preferences regarding hand sanitizers.
New and Returning Student Info
Make sure you have all the necessary paperwork ready for the upcoming school year. We provide handy checklists to ensure nothing is missed for both new enrollments and returning students.
When to Submit Forms
All required forms must be submitted before your child's first day of school. Action plan forms for specific health conditions should be submitted as soon as possible after enrollment.
